Well, technically you only need a DPST. But DPDT toggle switches are more common.

With a DPDT, each output wire (one +5V and one +12V) goes to each of the centre pins. Each input wire (one +5V and one +12V) goes to the relevant top or outer pin. The other two pins are ignored, as they are the off position.

With toggle switches, the idea is that the two nuts allow the front of the threaded part to be poking out of the panel just enough to fit the front nut. Then the rear nut tightens up to the panel.
