There was a similar bug in Watford's 8271 DFS. I reproduced it exactly when I worked there. The "solution" was to replace the complainant's ROM with an Acorn one. ![;)]( "Wink")
For years I've tried to remember the exact issue. It related to either a sequence of operations with no pause, or a sequence of operations with a long pause between. So, I think it related to the disk spinning down or not spinning down.
