If you end up needing 2x16k banks, it probably isn't worth special code for the B+ as the shadow ram + workspace + 8k iirc sideways ram use 32k and the 128 has proper banks so doesn't need special code, same with the master.

With the fire buttons, I read them every frame, stash them and or or then door depending on the routine to check if we have seen one and if not jump past the joystick routine. In another interrupt, but it could be the same after dealing with the fire buttons, I process the directions and start a new conversion.
