The timer reload takes 2 1mhz clocks.
Have you stopped the os from doing stuff and are you acknowledging the interrupt (I guess so).
If the os is still running, it is unlikely, but if an interrupt takes to long you might miss the odd one.
With your video, can you see that all pulses are the same length?
