Reading that fault description, it's almost certainly a bad solder joint on the LOPT or something else fairly chunky.

Even the likes of Sony were having problems with soldering large components reliably at the time - sometimes there'd be enough for a good connection, but not enough for mechanical stability.

The usual detection method was to look carefully at the joints - using a thin dye if necessary to highlight the cracks. Or if you didn't want to spend the diagnosis time, resolder all the LOPT and drive transistor pins, and anything else that looked suspicious...
